Here is something that you can use in your boothrental/chairrental/commission salon.
It is a great tool to get feedback from clients. It is also a great tool in getting feedback from Leasee's in your salon with just with a couple of question changes.

Client Questionnaire
Thank you for giving us the opportunity to serve you. Please help us better meet your needs by taking a moment to complete this questionnaire, honestly and completely.

1. How would you rate the salon? (Rate 1-5) (1____2____3____4___5____)
Poor Average Excels

2. How would you rate the phone etiquettes? (Rate 1-5)

3. Was our waiting room comfortable and clean? Yes / No (circle one)
If no, Why?

4. Do you feel our retail products are adequate for your needs? Yes/No
If no, Why?

5. Are the stylist/salon hour’s convenient for you? Yes / No

6. Do you feel like you can communicate openly with your stylist?
Yes / No

7. Do you feel the stylist is doing everything possible to fill your
needs with your hair concerns? Yes / No
If no, explain.

8. Does your stylist offer the option to color or perm your hair? Yes / No

9. Do you feel you have the option for a new hairstyle, if desired? Yes / No

10. Does the stylist explain what products she is using on you?
Yes / No

11. Do you have problems reaching someone at the salon to make appts.?
Yes / No

12. Do you book your next appt. before leaving the salon? Yes / No

13. Would you be interested in a Make-up and/or Facial cleansing line?

14. Would you be interested in having facials as a service?

14. Would you recommend our salon to your friends and family?

Sorry.. i dont. I run a BR salon and have contracts. Maybe someone else on here might be of some help.
